• Virtual dementia tour offered to GRECC-supported trainees and fellows through a partnership with the local Geriatric Workforce Enhancement Program (GWEP)

• Providing assistive listening devices in multiple VA clinics (e.g., primary care, emergency department) to support patient-provider communication and patient-directed care
• Routine gait speed evaluation to guide patient education and timely referrals, for example, to Fall Prevention Clinic and/or Gerofit
• Implementation of tele-palliative care and tele-dementia care (to Veteran’s home) to provide remote specialty services to older Veterans
• Multidisciplinary (geriatric medicine, physical and occupational therapy, and pharmacy) fall assessment and interventions to reduce the risk of falls in the community setting
