Case Management Week Spotlight: Cincinnati HUD-VASH Team

The VASH Case Management team has worked tirelessly to obtain and maintain housing for Veterans and provide supportive services in our 3 state, 11 county catchment area. All team members including SUD, Employment, Nursing, Case Aide, Housing Specialists, AMSA and OT, and they all contribute to the success of the Cincinnati VASH program. Our HCHV staff also stepped up and answered the call to the All Hands On Deck challenge.

What do you like about case management? How are you “Setting the Standard for Patient-Centered Care”?
VASH provides in-home case management to Veterans. All staff members have met the increasing needs of Veterans and have utilized ARP funds as needed to provide individualized resource management.
