Collaborative Case Management Guidance Updated
The National HUD-VASH
and SSVF Program Offices have developed updated guidance regarding
Collaborative Case Management. This guidance should assist communities to
understand and work towards collaborative case management models, versus
traditional and allowable co-enrollments between HUD-VASH and SSVF.
Please review the revised documents to assess local needs and collaborations
that will support Veterans with access to permanent housing within your
communities. If you have additional questions, please contact your Regional
Coordinator.

Grantee who is willing to share your knowledge with other grantees? Do you have
a best practice or specific set of skills that could benefit others?
SSVF Program Office is
looking for grantees to serve as mentors to their fellow grantees. Grantees
will be selected as general mentors as well as specialist mentors for specific
areas of need. If you are interested in this commitment of additional duties,
please complete this short interest survey. Please note, this survey does
not guarantee selection as a mentor grantee. The SSVF Program Office will
review submissions and contact you if you are selected as a mentor.
