With the aging of the veteran population there’s a growing demand to address multiple medical and mental health problem in a timely way. This model, integrating geriatrics and mental health imbedded in primary care, is uniquely suited to better manage all the Veteran’s medical and psychosocial needs. Social and psychiatric evaluations give a sense of the health risk, social support and physical limitations all within the primary care setting. (1.5 minutes)
