How to Report Your Financial Disclosure

Filing a confidential report: You must initially submit a New Entrant OGE Form 450 as part of your appointment. You will then annually file another New Entrant OGE Form 450 UNLESS VA has provided an alternative OGE-approved supplemental form to use in lieu of the OGE Form 450. As always, check with your VA ethics officials or Designated Federal Official (DFO) to find out which form you must submit.

The OGE Form 450 may be found on OGE's web site (Link to OGE financial disclosure forms on OGE's web site).

Send the completed report to the appropriate ethics official within your agency (not to OGE).
