F-R Formulary Restricted

This product is on the VA national formulary and has standardized restrictions such as being limited to a defined subspecialty of prescribers.

PA-F Prior Authorization - Facility

This product is on the VA national formulary and may have prior authorization requirements that are managed at the local facility level.

U/E Urgent/Emergent Formulary Item

This indicator in results lists is synonymous with the term 'U/E Formulary' and indicates that a drug is on the VA Urgent/Emergent formulary listing.

U/E* Urgent/Emergent Formulary item with limitations

The U/E indicator with asterisk indicates that a drug is on the VA Urgent/Emergent formulary listing with limitations. Open drug detail to see specifics of these limitations.

CFU : Criteria for Use

This term refers to specific inclusion and exclusion criteria and other clinical requirements for use of a given medication.

VISN : Veterans Integrated Service Network

This term refers to each of the 18 regional networks of VA facilities.

U/E Formulary : Urgent/Emergent Formulary Item

This term refers to the status of a drug on the VA Urgent/Emergent formulary listing. Items on the Urgent/Emergent formulary may be filled at outside pharmacies in some instances under specific provisions for Community Care.
